In most cases, you will have a set appointment time. Your scheduled time will be a standing appointment. Unless canceled at least 24 hours in advance, our policy is to charge for missed appointments at the rate of a normal office visit at our discretion.
This charge will be billed directly to the client.
Please help us serve you better by keeping scheduled appointments.
In case of inclement weather, please use your best judgment. We would rather you cancel your appointment than take unnecessary risks trying to get here.
COMMUNICABLE DISEASE MANAGEMENT POLICY
We want CNS to be a healthy environment for learning and rehabilitation. For this reason, we cannot treat clients who have symptoms of communicable disease. These symptoms include:
· fever of 100 Degrees F or higher
· diarrhea or vomiting
· severe coughing or difficulty breathing
· sore throat
· excessive nasal drainage which is not clear in color
· unusual rashes or untreated infected skin patches
· yellowish skin or eyes
· conjunctive pink/eye
· unusually dark urine or gray or white stool
· stiff neck
· evidence of lice, scabies, or other parasitic infestation
Clients should be free of symptoms for 24 hours before attending therapy. Please call the office as soon as possible if you will not be able to participate in therapy due to illness. We reserve the right to decline treatment to those who are exhibiting any of the above symptoms.
We are more than happy to share information regarding your progress with other professionals of your choice once we have your signed release form on file. Please refer to the Notice of Privacy Practices for more detailed information.
Siblings are always welcome at CNS! We ask that young children be supervised for their own safety.
BILLING AND PAYMENT POLICY
CNS gladly accepts cash, personal checks, American Express, MasterCard and Visa as methods of payment.
CNS will bill the appropriate insurance company when prior authorization has been obtained. All balances due are the responsibility of the policy holder.
Regulations stipulate that co- payments are to be paid at each visit.
If you are paying privately, charges are due at time of service.
If your appointment falls at a time when a receptionist is unavailable (evenings, weekends), please give your payment to your therapist.
If you need ongoing documentation of the amount you have paid, please let us know.
